This form must be filled out in its entirety and brought to the parade the day of (October 28th) at the registration table at MCU Park (1904 Surf Avenue) beginning at 9:45am.
Parent consent for the Coney Island Halloween Children’s Parade
I hereby give permission to my child to march in the Coney Island Halloween Parade on Saturday, October 28, 2017 on the Riegelmann Boardwalk from West 19th St. to West 10th St., Brooklyn, NY 11224. I also hereby consent to taking of photographs and videos associated with the Halloween Parade and the right to use such photographs and videos in all forms of media for the purposes of marketing the parade and/or the Alliance for Coney Island, a non-profit 501(c)3 organization. I understand that my child must wear closed shoes as they walk in the Parade. I release the Halloween Parade Committee, MCU Park, Brooklyn Baseball Co. LLC, the Alliance for Coney Island, and the City of New York from any and all responsibility or liability for injury to my child, should my child be injured while waiting at the assembly area to march in the Parade, or injured while marching in the Parade. I am responsible for escorting my child to the assembly area and for my child after the parade ends.
