Medication and Health Related Issues for September 2020

Dear PS 101 ParentS/Guardians:
Does your child need medication in school?
The 2020 – 2021 Diabetic/Medication Administration Forms (MAFs): asthma, allergies, general, 504 Forms, and admission forms are now posted on the DOE website.  These forms are available here https://infohub.nyced.org/in-our-schools/translations/forms/health-forms-and-notices.  There is a separate form for diabetics. The forms are needed to be filled by your medical health care provider each academic year.  Families can share this link with their health care providers.

Medical Administration Forms are to be sent to the attention of the below address:

Please remember to sign the consent form on the second page before mailing the forms and make a copy of the form for yourself to share with our School Nurse. Our School Nurse can assist you in September with medication intake and procedures.
