Dear PS 101 Families,

We need your celebrations! It is time to complete the NYC School Survey which celebrates PS101.

Completing the Survey is as easy as 1-2-3!

STEP 1- Complete the Parent Survey here (enter f and your child’s student ID/osis#)​​​​​​​: https://surveys.panoramaed.com/nycdoe/login
(Student ID numbers can be found on the top of the School Survey form or in the family’s NYCSA account.)

STEP 3: Your child will be rewarded with a FREE Full-Day NO Homework PASS

You must complete a survey for each child attending PS 101.

OPTIONS for completion:

Please have your child return the survey to their classroom teacher as soon as possible or kindly complete the survey on-line. 

1. Parents complete the paper survey, place it in the envelope and return it to your child’s teacher.
2. Parents may choose to complete online but must use their child’s student ID number. Parents will enter “f” followed by their child’s nine-digit Student Identification (OSIS) Number to access the survey. Note that “f” must be lowercase (f________).

Parent surveys must be completed and returned/submitted by March 31, 2023.

When you complete the survey, your child gets:

**Classes who have 100% completion will be awarded with the following reward:  1^st Class on the grade with full participation of returned surveys – ICE CREAM PARTY
