Dear PS 101 Families,
It’s Parent Survey time! School Survey = Your School + Your Voice!!! Please complete the Parent Survey to make sure your voice is heard and help us to celebrate PS 101! We care about our PS 101 Families!!!
You can complete the Parent Survey Online between February 12, 2024 – April 5, 2024 (enter f and your child’s student ID/OSIS#): https://surveys.panoramaed.com/nycdoe/login
(Student ID numbers can be found on the top of the School Survey form or in the family’s NYCSA account.)
OR
Complete the Paper Parent Survey we have sent home As Soon AS Possible! To ensure your paper surveys can be scanned properly, PLEASE:
- Use black ink, blue ink, or pencil for bubbling in responses.
- Fill in the selected answer bubbles completely. ( No Check mark or X mark, shade the bubbles)
- Do not include any stray marks on the page.
- Do not write over the QR code.
- Do not tear the paper or attempt to remove the staple.
Attention:
1. You must complete a survey for each child attending PS 101. (1 child= 1 survey; 2 children= 2 surveys… etc.)
2. When you complete the survey, your child gets a FREE Full-Day NO Homework PASS!!
3. Classes who have 100% completion will be awarded with the following reward: 1st Class on the grade with full participation of returned surveys – ICE CREAM PARTY!!!
